Extension of Probation Period Letter

Hi,
I am preparing a letter for the extension of his probation period. At the bottom of the letter, I am putting in this sentence..."I hereby confirm that I fully understood the decision and (accept/reject) the extension of the probationary offer."
Q1: By right, the staff have the right to accept / reject the extension?
Q2: What if the staff choose to reject the extension? What's next? Termination?

No need to give options for him to accept or reject. Just hand him the original copy of extension letter after informing him that his probationary period has been extended. Give him a pep talk about his weak points that he needs to improve etc etc. You may ask him to initial the copy as acknowledgment.
